Tacrine, an inhibitor of the enzyme acetylcholinesterase, entered the market in 1993 and was used with the scope to alleviate the symptoms. Despite the beneficial effects in the cognitive functions, tacrine was withdrawn two decades after its release due to severe hepatotoxicity. The molecular mechanisms underlying tacrine’s toxicity are still under investigation. In the present study, we focus on tacrine and its metabolites so as to investigate which one is responsible for the hepatotoxicity. Our primary aim is to delineate whether the 7-hydroxytacrine (7-OH THA) metabolite constitutes the toxicity culprit leading to the quinonation of liver proteins. Secondly, we test two novel derivatives of tacrine with a substitution in the C7 position, the 7-MEOTA and 7-FEOTA, so as to investigate whether these compounds could serve as safer alternatives to tacrine. Furthermore, the involvement of the toxic metabolite quinone methide in the hepatotoxic mechanism was assessed. For this purpose, the organotypic 3D model of primary human hepatocytes is used with the aim to recapitulate the in vivo situation more precisely. We conclude that the 7-OH metabolite is not the culprit for the hepatotoxicity and 7-FEOTA was the most toxic compound. Οι νευροεκφυλιστικές διαταραχές αποτελούν ένα από τα σημαντικότερα πρόβλημα υγείας, με τη νόσο του Αλτσχάιμερ να αποτελεί την πιο συχνή, πλήττοντας εκατομμύρια ανθρώπους παγκοσμίως. Η τακρίνη, ένας από τους αναστολείς του ενζύμου ακετυλοχολινεστεράση στο κεντρικό νευρικό σύστημα, εισήλθε στην αγορά το 1993 ως συμπτωματική θεραπεία της νόσου. Παρά τα ευεργετικά αποτελέσματα στις γνωστικές λειτουργίες, η τακρίνη αποσύρθηκε δύο δεκαετίες αργότερα, εξαιτίας σοβαρών περιστατικών ηπατοτοξικότητας. Μέχρι σήμερα, οι μοριακοί μηχανισμοί που διέπουν την τοξικότητα της τακρίνης βρίσκονται υπό διερεύνηση. Στην παρούσα μελέτη, εξετάζουμε την τακρίνη και τους μεταβολίτες της, προκειμένου να διερευνήσουμε τον υπαίτιο της ηπατοτοξικότητας. Πρωταρχικός στόχος, είναι να μελετήσουμε εάν ο μεταβολίτης 7-υδροξυτακρίνη (7-OH THA) εμπλέκεται στην τοξικότητα, ο οποίος διαταράσσει τη λειτουργία σημαντικών πρωτεϊνών του ήπατος. Σε δεύτερο επίπεδο, δύο νέες παράγωγες ενώσεις της τακρίνης, οι οποίες προκύπτουν έπειτα από υποκατάσταση της θέσης 7 με αιθερο-ομάδες, οι 7-μεθοξυ τακρίνη (7-MEOTA) και 7-φαινοξυ τακρίνη (7-FEOTA), εξετάζονται οι οποίες ίσως αποτελέσουν ασφαλέστερες εναλλακτικές θεραπείες έναντι της τακρίνης. Επιπλέον, εξετάζεται η εμπλοκή του τοξικού μεταβολίτη κινονο μεθιδίνη στον ηπατοτοξικό μηχανισμό. Για τη διεξαγωγή της συγκεκριμένης μελέτης, το τρισδιάστατο μοντέλο των πρωτογενών ανθρώπινων ηπατοκυττάρων χρησιμοποιήθηκε το οποίο προσομοιάζει τις in vivo συνθήκες.
